There are many options available to breast augmentation patients, including the size and shape of the implant being used and the material it is made of. The size and location of incisions and the final location of the implants during breast augmentation surgery will depend on these factors and the preferences of each patient.
One of the most important choices facing breast augmentation patients is whether to have the implants placed beneath or above the chest muscles. Patients who choose to have the implants placed beneath the chest muscles may enjoy a somewhat more natural feel and slope of the breast, but they also face more post-operative pain and a longer recovery time as muscles in the chest adapt to their new position. Meanwhile, patients who chose implants placed above the muscles enjoy a less invasive procedure, less post-operative pain, and a shorter recovery time.
Most implants are placed via the inframammary fold under the breast. Inframammary breast augmentation allows Dr. Glat the best control during surgery, but does cause a small scar at the base of the breast that may be visible when the patient is lying down. One alternative is to make an incision in the armpit. Patients who choose this option are able to avoid scarring on the breast itself. However, because the incision is made considerably further away from the breast, this technique does not allow for as much control over the procedure as the inframammary incision does.
Patients who want more discreet incisions can choose to have breast augmentation via incisions around the nipple or in the navel instead. Incisions around the nipple typically heal very well and are difficult to detect after the surgery. Incisions in the navel are the hardest to detect after healing. However, only saline implants can be placed through the navel.
Today, women who are interested in breast augmentation are able to choose between silicone and saline implants. Both are FDA-approved for safety, and each has significant advantages. Saline implants are the safest option because saline is non-toxic and can be easily absorbed by the body in the event of a rupture. If a saline implant does rupture, it is quickly evident because the implant deflates almost immediately. While silicone implants are also safe, and ruptures are very uncommon, silicone may become a health risk if a rupture does occur.
The newest saline implants have a very realistic look and texture, but many breast augmentation patients feel that silicone implants look and feel even more realistic. On the other hand, because saline implants are filled after implantation while silicone implants must be filled before the surgery, the incisions for breast augmentation with saline implants are typically smaller.

Patients should expect soreness for some time after surgery, and will not be able to perform strenuous physical activities for about a month.
